Stakeholder Group Charter
Who:
Kane Health Counts Stakeholders; individuals representing a wide-variety of sectors in the community who contribute to the public’s health and well-being.
Purpose:
Kane Health Counts Stakeholders serving Kane County are committed to working together to help Kane County residents achieve optimal health and overall well-being.
The Stakeholder Group is convened by the Kane County Health Department to participate in assessment and planning activities for the Kane County Community Health Assessment (CHA) and the Community Health Improvement Plan (CHIP). As priorities emerge from the assessment data, stakeholders will work together to plan, implement and evaluate strategies that are in alignment with the mission and activities of the stakeholder’s organization, institution and group.
Methods:
The process to complete the CHA and CHIP is the Mobilizing for Action through Planning and Partnerships (MAPP) process. The Kane Health Counts Executive Committee provides guidance and oversight to the community assessment, plan, implementation and evaluation.
When:
During implementation the Stakeholder group will meet at least annually to share their progress. Implementation of action plans will begin in 2021 and conclude in 2024 for the next community assessment.
